A referral means moving another person's medical records across a border. That belongs behind authentication with consent recorded against it, not in a web form on a marketing page — so the button below takes you to a signed-in workspace and nothing is transmitted before then.

Please do not send records by email either. If you have already emailed something, tell us and we will confirm its deletion.

Have these ready

None of this is needed to start a conversation about whether a pathway is open. It is needed before a case can go to a hospital.

  • Your patient's consentRecorded in the workspace, naming the receiving institution and scoped to this referral. Your patient can withdraw it, which stops further sharing.
  • A short clinical summaryIndication, stage or classification, prior lines of therapy and response, current medications, and performance status. A paragraph is enough to begin.
  • Imaging and pathologyReports at minimum; source imaging where a hospital requires re-reading. We tell you which applies before you collect anything.
  • Your licence detailsRegistering body and registration number in your jurisdiction. We verify this before a case is submitted, because a hospital is being asked to act on a referral from a named physician.
  • How you want the responseWhether the written response goes to you, to your patient, or to both. This is set per referral and can be changed.

Before you start

Can I ask about a pathway without submitting a case?
Yes, and it is the better first step. Availability and regulatory status can be answered without any patient information at all.
What if my patient has not consented yet?
Start anyway. Consent is recorded at the point records are attached, not at signup, so you can scope the referral first and involve your patient once you know something is actually open.
Who can see the case?
You, the coordinator assigned to it, and — once you approve the packet — the receiving hospital. Access is per-case and can be withdrawn.
Can I refer more than one patient?
Yes. Cases are held separately, with their own consent and their own access, under one practice workspace.
